When you are finished, click the Submit Faith Promise button at the bottom of the form.   Your on-line Faith Promise will be sent right to us electronically.   Note:  this site is not equipped to transfer funds.   Once you submit the form, you may send to us your check for the first payment (if applicable) of your Faith Promise, payable to CPCI, Inc. via postal mail to:
